779.04   APPLICATION FOR PUBLIC VEHICLE LICENSE.
   Every applicant for a license to operate a public vehicle shall file a written application with the Director of Public Safety containing the following information:
   (a)   The full name, age and residence of the applicant, and, if the applicant is a partnership, the full name, age and residence of each of the partners, and, if a corporation, the full name of the corporation, together with full names of its principal officers.
   (b)   The name under which the applicant will do business.
   (c)   The type of vehicle which is proposed to be used in the service, with information as to the age, carrying capacity and motor power of same; the color scheme, local symbols to be used, together with all other lettering or marks proposed to be used by the applicant on the vehicles.
   (d)   The number of vehicles proposed to be used and the rate of fare to be charged and the method of charging the same.
   (e)   The applicant is capable of and will carry liability insurance or bond or agreement, as hereinafter provided for.
   (f)   The application shall be accompanied with a fee as set out in Chapter 185 of the Administrative Code (the General Fee Schedule) to cover the amount of expense that Council deems the City will incur in investigating the application and serving notice of the required public hearing.
