753.03   PERMIT APPLICATIONS.
   Any person desiring to engage in the drilling or operation of a new well for gas or oil, or the deepening or modification of any new or existing well for gas or oil, or the site upon which such a well is located, shall, prior to the commencement of any operation associated with such activity, make an application therefor to the Director of Public Safety. Applications shall be filed by the owner of the property upon which such a well is located or by a person who has been granted authority by the owner to file such application. There shall be a separate application filed and permit required for each proposed well. Such application shall consist of the following:
   (a)   A copy of an approved permit application from the Division of Oil and Gas of the Ohio Department of Natural Resources. The application shall include all information required therein along with evidence of compliance with all conditions for such permit approval.
   (b)   Eight copies of a plat drawn by a registered surveyor to a scale not smaller than 100 feet to an inch, or as required by the City Engineer, and indicating:
      (1)   Proposed locations of the well, tanks and other large equipment, access drives, pipelines, fences, landscaping and any culverts, ditching or other physical changes to the existing drainage or topography of the site;
      (2)   Locations, uses and ownership of all land, buildings and structures, parking areas, drives, public rights-of-way, property lines, zoning and drilling unit boundaries within the drilling unit, and all utility easements within 1,000 feet of the minimum drilling unit area required;
      (3)   The topography of the site and abutting properties;
      (4)   The designated route for City inspection and emergency access, proposed truck routes in and out of the site via public roads and proposed truck routes to the site for such time such trucks are within the City. The latter route may be provided at a scale approved by the City Engineer.
      (5)   Necessary dimensions to indicate compliance with all minimum distances required in this section;
   (c)   The name, address and telephone number of a person responsible for the operation and maintenance of the site and available for contact for emergency or other purposes;
   (d)   A plan showing all pipelines, connections and shut-off valves for emergency purposes;
   (e)   A fee as set forth in Chapter 185 of the Administrative Code (the General Fee Schedule); and
   (f)   Any other information necessary to adequately determine compliance with this chapter or State law.
