(a) An applicant for an itinerant vendor's license shall file an application for such license, which shall provide to the Director of Public Safety the following information:
(1) The full name of the individual;
(2) The address of the individual;
(3) The birth date, Social Security number, height, weight and color of eyes and hair of the individual, and whether such individual has ever been convicted of any crime and, if so, the nature thereof and the penalties therefor;
(4) The location he or she plans to sell from and the written authorization of the property owner evidencing permission for the applicant to be on the property and for how long;
(5) Proof that the authorizing property owner has premises liability insurance under which the operation of any itinerant vendors upon such premises shall also be so insured;
(6) Whether the individual has complied with State law relative to obtaining a State license as an itinerant vendor, if compliance is applicable and necessary.
(b) Upon completion of the application described in subsection (a) hereof, the Director shall forthwith issue an itinerant vendor's license, which shall expire after thirty days.
(c) Upon issuance of the itinerant vendor's license, the grantee shall pay a fee as set forth in Chapter 185 of the Administrative Code (the General Fee Schedule).
(d) A copy of the applicable itinerant vendor laws shall be given to the vendor upon issuance of the itinerant vendor's license.
