175.02   IDENTIFICATION OF PERSONAL INFORMATION SYSTEMS.
   (a)   Every City Administrator shall annually survey all records which are maintained in his office and identify those records which constitute a personal information system. On or before November 1 of each year, a Notice of Personal Information System shall be completed for each system identified and shall be forwarded to the office of the Mayor.
   (b)   The Mayor, on or before November 30 of each year, shall keep a record of the Notices of Personal Information Systems which he receives and shall compile and submit them to the Ohio Privacy Board on behalf of the City.
