157.06 INSURANCE.
   Hospital and medical insurance as well as dental, vision and life insurance shall be carried for full-time employees by the Village. Family hospital and medical insurance as well as dental and vision insurance shall be available at an annual cost to the employee of 2.5% of the additional premium. The Village has implemented an employed spouse rule starting with the 2022 plan year. This rule includes the following stipulations:
   (a)   If an employee’s spouse is eligible to participate in any group health insurance plan sponsored by his/her employer, including a public or private retiree health plan, your spouse will be required to enroll in that employer sponsored health insurance plan as primary coverage.
   (b)   If an employee elects to continue a spouse’s coverage through the Village plan (after the spouse enrolls in his/her employer’s plan), the Village plan will be secondary. Additionally, the employee will be charged 100% of the monthly rate billed to the Village for this coverage.
   (c)   The certification form titled, “Spouse Benefit Information” is to be completed and signed by the Village employee and the employee’s spouse’s employer. Failure to return this form to the Village will result in the employee being charged 100% of the premium for the spouse on the plan until the form is returned.
   (d)   This rule only requires spouses to enroll in his/her employer’s health plan; children may remain on the Village plan.
   (e)   It is an employee’s responsibility to immediately notify the Clerk’s Office of a change in a spouse’s eligibility to participate in his/her group health insurance plan.                     
   Elected officials are not eligible for insurance coverage through the Village of Ottawa.
