(a) Each employee shall be entitled for each completed eighty hours of service, to sick leave of four and six-tenths hours with pay, which sick leave shall be accumulated without limit. Employees may use sick leave upon the written permission of the department head for absence due to personal illness or injury and for illness, injury or death in the employee’s immediate family. The written permission form shall be filed in the employee’s personal file in the Clerk’s Office. Immediate family for sick leave purposes shall be defined as husband, wife, son, daughter and other persons at the discretion of the Mayor.

(b) Any employee who is absent for more than two consecutive work days shall present to the department head a doctor’s statement indicating the nature of his illness or injury and the projected date of his return to work, together with any conditions imposed by the doctor for his return to work. The doctor’s certificate shall be presented to the department head no later than the sixth consecutive work day after the commencement of the employee’s absence or on the third day of the employee’s absence or upon the return of the employee to work, whichever is sooner. This rule may be temporarily waived by the Mayor in exceptional circumstances where the employee or a member of his family is unable to provide the certificate for reasons beyond his control.
