(a) An application for a storm sewer connection permit shall be signed by either the owner or agent of the property for which the connection is to be made, and by the person employed to perform the work of installing the storm and/or storm sewer service connection. The application shall include the information required by Chapter 1481.
(b) All single family, single family dwellings, whether or not located on one lot, shall each have separate connections to, and separate lines from, the main storm sewer line to the existing dwelling.
(c) All connection laterals shall be constructed directly from the existing building and/or structure.
