No picnic, public gathering, entertainment or other similar event shall be held upon any premises licensed under this chapter unless a permit therefor has previously been secured from the Director of Finance, which permit shall be based upon a written application therefor, as follows:
APPLICATION FOR PERMIT
To the Director of Finance,
Municipality of North Royalton, Ohio.
The undersigned hereby makes application for a permit to hold a public picnic or other entertainment as follows: (blank space for recital of purpose), at on the day of 19 between the hours of m. and m., and agrees to abide by all the provisions of the Codified Ordinances of North Royalton providing for the licensing of such premises and governing the issuance of this permit and agrees that the same shall be held in conformity with the provisions of all laws.
