(a) An abortion service shall keep records, including admission and discharge notes, histories, results of tests and examinations, nurses' work sheets, social service records and other progress notes of a patient, and it shall submit such records to the State Department of Health if such Department so requires. Such records will also include written verification by one or more physicians of the diagnosis and duration of pregnancy.
(b) Whoever violates or fails to comply with this section is guilty of a misdemeanor of the first degree. Punishment shall be as provided in Section 698.02.
