Every person, society, club, firm, organization, corporation or other legal entity, desiring a license to operate a teen dance club must make personal application to the Director of Public Safety for a license. Each application shall contain the information set forth below in Section 763.07 and shall be on a form prescribed by the Director. The Director shall issue the license within twenty days following the date the application is received, unless cause exists as provided in Section 763.09 below to deny the license. Upon receipt a license to operate a teen dance club, the owner or operator shall post the license in a conspicuous place at the teen dance club. (Ord. 2006-48. Passed 3-7-06.)
Every applicant for a teen dance club license shall submit a written application stating the name and complete address, including nine digit zip code of the applicant, or if more than one person or if an association or firm, the full name of all parties interested and the complete address including nine digit zip code. If the applicant is a club, firm, society, organization or corporation, the applicant shall contain a complete list of the officers of such club, society, organization, firm, or corporation, with their names and addresses, including nine digit zip code, and shall also give the state in which such club, society, organization firm, or corporation is organized, its federal employer identification number (EIN), and the names of one or more persons whom the club, society, firm, organization or corporation desires to designate as its manager or managers, or person or persons, in charge with their complete addresses. The application shall also state the following:
(a) The location where such teen dance club is to be operated, specifically, complete address and nine digit zip code for said location.
(b) If the applicant is an individual, the name, address, social security number, and date of birth of the applicant. If the applicant is a club, society, firm, organization, corporation or group of individuals, the names, addresses, social security numbers, and dates of birth of the officers, managers and/or members of said organization.
(c) Whether the applicant (or any person listed on the application) has engaged in operating any dance hall or other teen dance club in the past five years, and when, where and how long in each place.
(d) The name of the owner of the premises in which such teen dance club is to be operated, and the complete address of such owner.
(e) The names, addresses, social security numbers, dates of birth and signatures of each person who will be serving as a “supervising adult or chaperon” as required by Section 763.14 below. If the owner of the premises rents the hall or club to an individual or organization who intends to use same for a teen dance, the owner of the premises shall notify the Director of Public Safety of the fact of the rental and purpose of the event and designate an individual, or, in the case of an organization, a manager or one of its principal officers who shall provide the information required by this section and listed above.
(f) The application shall be signed by the applicant or applicants, or in case of a club, society, firm, organization or corporation, the application shall be signed by the manager or one of its principal officers. Each person whose name is listed on the application for license or notification of designee in any capacity shall sign an authorization for a criminal record check by the City.
(g) A completed Ohio Bureau of Criminal Identification and Investigation (BCI) background check, obtained at applicant’s cost, shall accompany the application. The BCI background check must be of the applicant, and ay adult supervisor or chaperon if named in the application. If the application is for an individual, and must be for all officers, managers, and/or members, and any adult supervisor or chaperon named in the application, if the applicant is a club, society, firm, organization, corporation or other group of individuals.

The Director of Public Safety shall require the Chief of Police to conduct, or assign to a member of the Police Division to conduct, an investigation of each application for a teen dance club license and determine whether the application complies with all of the regulations, ordinances and laws applicable thereto, and shall cause an investigation as to the character and fitness of the applicant or applicants, or the officers of the club, society, firm, organization or corporation (including each person listed on the application) and of the person, who is to have the general management of such teen dance club, as well as an investigation of all persons listed as “supervising adults or chaperons” for teen dances.
