145.16.4 INCIDENT REPORTS.

Any employee of the Village involved in an incident which results in property damage or personal injury shall report such incident to the Mayor as soon as practicable on the Incident Report form attached to original Resolution 143.548, passed July 6, 1987, or its future substitute form. (Ord. 143.548. Passed 7-6-87.)
