Personnel for the Village of North Kingsville:
(a) Will not use or further disclose protected health information other than as permitted or required by the plan document or as required by law.
(b) Will ensure that any subcontractors or agents that the Village uses will abide by the same restrictions as contained herein.
(c) Will not use or disclose protected health information for employment-related actions.
(d) Will report any use or disclosure of protected health information that is inconsistent with the health insurance plan document.
(e) Will make the protected health information accessible to the individuals covered under the plan.
(f) Will allow the individual to amend the protected health information.
(g) Will provide an accounting of all disclosures to the individuals covered under the plan.
(h) Will make its practices available to the Department of Health and Human Services for determination of compliance.
(i) Will return or destroy any protected health information no longer needed, if feasible.
