145.15 .1 DISCLOSURE OF PROTECTED HEALTH INFORMATION.

   Protected health information may only be released by personnel of the Village of North Kingsville under the following three circumstances:

First: The individual health insurance member authorizes, in writing, the release of such information.

Second: The protected health information is being used solely to obtain premium bids or to modify, amend or terminate the group health insurance plan.

Third: The protected health information is being used solely to amend health insurance plan documents.
