§ 35.604 APPLICATION FOR LEAVE OF ABSENCE.
   Any employee who wants to go on a leave of absence must apply in writing to the Municipal Manager and include all necessary information as to length of absence, starting date, reason, and other pertinent details. This will submitted to the employee's department head as early as possible prior to the start of the requested leave. Employees requesting military, maternity, personal, or educational leaves of absence are requested to submit an application at least four weeks prior to the start of such leave. Required municipal training, funeral leave, sickness, or non-job-related injuries requiring absences of no more than 14 calendar days are not subject to these guidelines for applications for leaves. Temporary and provisional part-time employees, as well as probationary employees, are not eligible for any leave of absence, unless otherwise required by law.
