Applicants for permits and licenses under this chapter must hie with the Village Administrator a sworn application, in writing, on a form to be furnished by the Village Administrator, which applicant shall give the following information:
(a) The name and a physical description of the applicant including a current photo ID;
(b) The address and telephone number of the applicant;
(c) A brief description of the nature of the business and the goods to be sold and, in the case of products of farm or orchard, a statement as to whether or not such products were produced or grown by the applicant;
(d) If employed, the name and address of the employer;
(e) The length of time for which the right to do business is desired;
(f) If requesting a license and permit for a mobile food service operation, proof of licensing with the Health Department and payment of the fee, or proof of a statement of exemption;
(g) In the case of all persons who go door to door and who use a vehicle for transportation, proof of having a valid Ohio driver's license and adequate liability insurance for the operation of such vehicle. A minimum of three hundred thousand dollars ($300,000) in liability insurance shall be required; and
(h) Applicant must be at least 18 years of age.
