§ 31.01 IMMUNIZATION.
   (A)   The County Health Officer, his or her aides and assistants shall provide, at public expense, shots and immunizations for Scarlet Fever, Rabies, Tetanus, Diphtheria and such other similar antitoxin shots, and immunization as the County Public Health Officer may by law be required to furnish.
   (B)   Along with such antitoxin, immunization and shots mentioned aforesaid, the County Health Officer, his or her aides, and assistants shall furnish insulin subject to the same standard of indigency as provided herewith.
   (C)   The criteria for determining indigency shall be the standard upon which the State Food Stamp Program is based, a copy of such standard is attached to the ordinance from which this section is derived as Exhibit A and hereby made a part of this section, such standard indicating the maximum monthly income whereby a person may qualify for such assistance. Any such person applying for such immunization, antitoxin, or similar shots enumerated aforesaid shall be advised of such standard and if they qualify they shall sign an affidavit stating their eligibility prior to receiving such immunization, antitoxin, or similar shots. The County Attorney shall furnish to the County Health Officer forms of affidavits to be filed by persons claiming indigency for the purposes described herein.
   (D)   The County Health Officer, his or her aides, and assistants shall only give such shots and immunizations when such is recommended by a physician who is licensed to practice medicine in the state.
   (E)   In event the requirement for rabies antitoxin results from a dog bite the cost for such shot shall be billed to the Township Trustee of the township in which the dog bite occurred.
   (F)   Such requirement for indigency as heretofore set forth shall be modified at such time and in the same manner as the State Department of Public Welfare or its successors shall modify the maximum allowable net food stamp income for non-public assistance households.
