§ 111.03 APPLICATION FOR PERMIT OR LICENSE.
   All applications for a permit or annual license required by this chapter shall be in writing on a form designated by the Commissioners, and shall contain, at a minimum, the following information and be signed by the applicant:
   (A)   The name, mailing address and telephone number of the applicant, and the names and addresses of all partners if a partnership, all officers of a corporation, and all other persons who will be associated in the operation of the business;
   (B)   For dance hall permit applications, the date and hours of operation, the street address of the location of the dance, and the date of the dance;
   (C)   For annual dance hall license applications, the location (street address), hours of operation, the maximum number of days per week that dances will be held, the zoning designation of the property, and the capacity of the dance hall as determined by the State Fire Marshal or local Fire Chief;
   (D)   Whether the dance or dances will be open to the public, and whether there will be an admission charge, or any age or other restrictions on who may be admitted;
   (E)   Whether the conditions described under § 111.05 of this chapter applies to the applicant, or partners in a partnership or officers of a corporation operating or owning the public dance facility; and
   (F)   Any other information deemed appropriate by the Commissioners including, but not limited to, that which is required by this or any other ordinance, law or regulation.
