4-17-2: APPLICATION AND APPLICATION FEE:
Every applicant for a license to maintain, operate or conduct a massage establishment shall file an application for a massage establishment license with the village clerk upon a form provided by the village clerk and shall pay a nonrefundable application fee of fifty dollars ($50.00) for an original or renewal application.
