139.06 HOSPITALIZATION.
   (a)   Generally.
      (1)   The City will provide hospitalization for full-time employees and elected officials of the City, except members of the Police and Fire Divisions.
      (2)   The service contracted for shall be semi-private service and shall extend to the above stated employees, elected officials and their families. Effective January 1, 1982, the hospitalization insurance provided for shall be modified to provide the prescription drug rider benefit.
      (3)   Such hospitalization shall be available to the full-time employees and elected officials of the City mentioned in subparagraph (1) hereof immediately upon entering into service with the City.
      (4)   The Mayor is hereby authorized to enter into any contract necessary to carry out the terms of this subsection.
      (5)   The Director of Finance is hereby authorized to pay the premiums as the same become due for the service, and the payments for the premiums are hereby appropriated from the General Fund.

   (b)   Police and Fire Division Personnel.
      (1)   Effective June 1, 1971, the City will contract with Blue Cross of Northeast Ohio to provide for hospitalization for full-time members of the Police and Fire Divisions.
      (2)   The service contracted for shall be semi-private service and related medical-surgical service, and shall extend to the members of the Divisions and their families.
      (3)   The Mayor is hereby authorized to enter into any contract necessary to carry out the terms of this division.
      (4)   The Director of Finance is hereby authorized to pay the premiums as the same become due for the service, and the payments for the premiums are hereby appropriated from the General Fund.

   (c)   Non-bargaining Unit Employees. All full-time employees and elected officials of the City shall receive, at no cost to the employee or elected official, such hospitalization, medical, dental, life insurance and prescription benefits as determined by the City. Full-time employees or elected officials who are eligible may “opt out” of:
      (1)   Hospitalization, medical, prescription benefits and receive thirty-five dollars ($35.00) per month for family coverage or fourteen dollars ($14.00) per month for single coverage;
      (2)   Hospitalization, medical, prescription benefits, dental benefits, and receive eighty-five dollars ($85.00) per month for family coverage or forty-three dollars ($43.00) per month for single coverage;
      (3)   Hospitalization, medical, prescription benefits, vision benefits and receive fifty dollars ($50.00) per month for family coverage or twenty- five dollars ($25.00) per month for single coverage;
      (4)   Hospitalization, medical, prescription benefits, dental benefits, vision benefits and receive one hundred dollars ($100.00) per month for family coverage or fifty dollars ($50.00) per month for single coverage.
