(a) Scope. Records shall be kept of every blast in an approved manner as the Fire Chief may require.
(b) Minimum Recorded Data. Minimum recorded data shall be as follows:
(1) The plan of the involved portion of the construction project showing the location and spacing of holes;
* (2) A cross-section of each hole showing overburden: burden at top of face and toe; height of face; depth of hole; kind and quantity of explosives, including distribution of explosives load as deck charges, or otherwise as appropriate; and length and kind of stemming material;
(3) The drilling record showing unusual joint or seam conditions in the rock;
(4) The kind of blasting caps; the distribution of instantaneous or short period delay blasting caps, or both; the delay interval used; and the order of firing;
* (5) The comments by the blaster in charge regarding the character of breakage; the height of breakage; the length of throw; and the effectiveness of shot;
(6) The comments by the blaster regarding unusual results or effects;
(7) The weather conditions including the direction and approximate velocity of wind; atmospheric temperature; relative humidity; and the cloud conditions;
(8) The date and time of firing of the blast;
(9) The name of the person responsible and in charge of loading and firing and the blaster permit number:
(10) The name and the location of the construction project; and
(11) The signature and the title of the person making the report.
* Indicates data required for side-hill or through cut construction only.
(c) Number of Copies. At least one copy of the records required shall be maintained at the site at all times and two copies shall be delivered to the Fire Chief.
(d) Inventory. A continuous inventory of explosives purchased for any given construction project shall be kept continuously at the site. Such inventory shall include materials on hand, materials received (from purchase, returns to storage or otherwise), materials charged out, the name of the storekeeper and dates.
(e) Seismograph Records. If the seismograph or air pressure readings, or both, are taken upon authorization of the contractor, two copies of the covering seismograph report shall be forwarded to the Fire Chief without delay together with copies of the records required to be maintained by subsection (c) hereof in order to provide basic data for research and study of seismic effects.
(f) Recorded Seismograph Data. Seismograph reports shall, at a minimum, include identification of the instrument; the name of the observer; the name of the interpreter; the distance and direction of the recording station from the area of detonation, the type of ground at the recording station or the location of the structure; the maximum amplitudes for all components as well as resultant for all recorded frequencies of vibrations; the duration of motion in excess of one-one thousandth of an inch; and a copy of the photographic record of the seismograph readings.
(g) Qualifications of Seismologist. Seismograph reports shall be signed by a person whose qualifications are satisfactory to the Fire Chief and City Engineer. Such person shall be held responsible for the authenticity and correctness of such reports.
