§ 114.27 APPLICATION.
   (A)   An application for a food purveyor's license shall be made in the office of the City Clerk upon the forms there at available for such purpose. The application shall contain such further information as may be required by the Board of Health in order to inform it fully as to the size and nature of the place to be used for the purposes of the business and the conditions, equipment, and facilities for conducting the business therein, the health of persons to be employed, and the regulations and facilities provided for them.
   (B)   Before any license shall be issued, the Board of Health shall cause an inspection to be made of the premises to be occupied by the applicant as a food purveyor, the equipment contained therein, the employees intended for the same, and all other matters relating to the business which it considers necessary to investigate from a health or sanitary standpoint.
