(a) An application for a certificate of authorized occupancy required by this Housing Code shall be made to the Director of Buildings on forms supplied by him or her. Immediately upon the receipt of such application, the Director of Buildings shall cause a general inspection of the premises to be made.
(b) Upon the completion of the inspection, the Director of Buildings shall issue a certificate of authorized occupancy for such premises which shall contain the following information:
(1) The street address or other identifying characteristics of the premises;
(2) The name and address of the owner and of the agent;
(3) The authorized use and occupancy of such premises; and
(4) Reference to the other requirements of this Housing Code and to the available City housing records and inspection service.
The certificate shall be valid until a change or alteration of the premises alters the authorized occupancy or until there is a change in the ownership of the premises.
