§ 165.05 INSURANCE.
   (a)   All regular full-time employees, except those covered under collective bargaining agreements, and the Judge of the Mason Municipal Court, are covered by the following medical and life insurance program provided for employees of the city.
      (1)   Medical insurance for employees and dependent.
         A.   Medical insurance for each eligible employee and eligible dependents of the employee will be provided. The amounts and method of coverage will be determined by Council.
         B.   1.   The City Manager hereby is authorized to enter into a contract to provide medical insurance benefits to all part-time and temporary employees of the city.
            2.   The medical insurance benefits will be as generally outlined in the attached Exhibit A, attached to original Ordinance 2007-158 which is incorporated herein by reference.
            3.   All part-time and temporary employees of the city desiring to obtain medical insurance benefits shall be solely responsible for all costs of premiums of the medical insurance benefits.
      (2)   Life insurance. 
         A.   Effective January 1, 2005, $50,000 death benefit with accidental death and dismemberment provisions or endorsement as may be attached thereto, for all covered employees except as provided in B. below.
         B.   Effective January 1, 2005, $100,000 death benefit with accidental death and dismemberment provisions or endorsement as may be attached thereto for the City Manager, Assistant City Manager and all Department Heads.
         C.   Twenty-five thousand dollars death benefit with accidental death and dismemberment provisions or endorsement as may be attached thereto, effective until January 1, 2005, for non-represented, non-administrative employees, and $50,000 for the City Manager, Assistant City Manager and all department heads effective until January 1, 2005.
      (3)   Dental insurance. Dental insurance for each eligible employee and eligible dependents of the employee will be provided. The amounts and method of coverage will be determined by Council.
      (4)   Supplemental psychological counseling benefits. Supplemental psychological counseling benefits.
      (5)   Optical insurance. Optical insurance for each eligible employee and eligible dependents of the employee will be provided. The amounts and method of coverage will be determined by Council.
      (6)   Disability insurance. Disability insurance for each eligible employee will be provided. The amounts and method of coverage will be determined by Council.
   (b)   New employees are eligible to receive these benefits after completion of 30 days’ employment.
