Sec. 58-62. Application.
Application for permits required in this division shall be made upon forms provided by the zoning official and when submitted shall contain or have attached the following information:
   (1)   Name, address, telephone number and, if applicable, facsimile telephone number and email address, of the applicant.
   (2)   Location of the building, structure and parcel to which the sign is to be attached or erected.
   (3)   Position of the sign in relation to nearby buildings, structures and property lines and existing or proposed rights-of-way.
   (4)   Two copies of the following: site plan drawings, sign plans and specifications, illumination specifications, materials description, and method of construction and attachment to the building or in the ground.
   (5)   Copy of stress sheets and calculations showing the structure as designed for dead load and wind pressure, if deemed necessary by the zoning official.
   (6)   Name, address, and telephone number of the person erecting the sign.
   (7)   Evidence of liability insurance as required by section 58-63.
   (8)   Such other information as the zoning official may require to show full compliance with this chapter and all other applicable laws of the city and this state.
   (9)   If the applicant is anyone other than the property owner, the request must be accompanied by a signed letter of authorization from the property owner.
