1301.14 LICENSE AND PERMIT APPLICATIONS.
   (a)    All original applications for licenses and permits, unless otherwise specially provided, shall contain the following minimum information, in writing, upon forms provided by the Bureau:
      (1)    Name of the applicant and business associate, partner or chief officer and trade or business name.
      (2)    Representative capacity of applicant, if applicable.
      (3)    Place of residence and place of business.
      (4)    Present trade, occupation or business phone number.
      (5)    Date of application.
      (6)    Time duration of permit or license.
      (7)    If for a vendor, description of merchandise to be sold.
   (b)    Renewal of an annual license or permit may be granted to one in good standing upon the original application, unless otherwise provided.
   (c)    The applicant shall pay the required fee at the time of making original application and upon each renewal application.
   (d)    All license and permit fees, unless otherwise provided, shall be paid into the City Treasury to the credit of the General Fund.
   (e)    The Bureau may, by rule, prescribe that annual permits and licenses shall be valid for a term of one year other than the calendar year. If so established, provision shall be made by the Bureau to prorate the required fee to insure that no fee is twice paid for the same period of time.
   (f)    No person shall knowingly make any false statement or representation in any application for a license or permit, and the same, in addition to other penalties provided by law, shall be reason to refuse or revoke the license or permit .
   (g)    If for any reason the license or permit is not issued, the fee deposited by the applicant shall be returned to him, less fifty percent (50%) thereof as a service charge; provided that such service charge shall not exceed five dollars ($5.00) nor be under one dollar ($1.00).
