1161.04 APPLICATION PROCEDURE.
   Any person or persons desiring an amendment of, or supplement to, the Zoning Ordinance or Zoning Map shall file an application for such amendment with the Clerk of Council. The application shall contain at least the following information:
   (a)   Name, address and phone number of applicant;
   (b)   Present use of the property;
   (c)   Present zoning district;
   (d)   Proposed use of the property;
   (e)   Proposed zoning district;
   (f)   A vicinity map showing property lines, thoroughfares, existing and proposed zoning of adjacent parcels and of those across the street.
   (g)   A list of all property owners and their mailing addresses who are within, contiguous to or directly across the street from the parcel(s) proposed to be rezoned, except that names and addresses need not be included where more than ten parcels are to be rezoned.
   (h)   A statement on how the proposed amendment relates to the City's Comprehensive Plan.
   (i)   A statement giving evidence as to why this Ordinance and Map are unreasonable with regard to the particular property in question and deprive the property owner of the lawful and reasonable use of his land.
   (j)   A nonrefundable application fee of two hundred and fifty dollars ($250.00) to cover all advertising and processing costs.
