(a) The charge for basic life support shall be $900.
(b) The charge for advanced life support 1 shall be $1,150.
(c) The charge for advanced life support 2 shall be $1,250.
(d) The charge for non-transportation shall be $175.
(e) The charge for mileage shall be $18 per mile.
(f) The charge for a motor vehicle accident scene safety and investigation shall be $435.
(g) The charge for a motor vehicle accident hazardous material response shall be $496.
(h) The charge for a motor vehicle accident with entrapment shall be $1,800.
(i) The charge for use of an air transport shall be an additional $300.
