926.09 APPLICATION REQUIREMENTS.
   (a)   Unless specifically excluded by this regulation, any land owner or operator desiring a permit for a land disturbance activity shall submit to the City of Loveland a permit application on a form provided for that purpose.
   (b)   Unless otherwise excepted by this regulation, a permit application must be accompanied by the following, as applicable, in order that the permit application be considered: a storm water management plan; an operation and maintenance agreement; easement(s) for post-construction storm water quantity and/or quality treatment basins and devices; a storm water pollution prevention plan; a copy of the approved OEPA General Construction Permit; and a non-refundable permit review fee.
   (c)   The storm water management plan shall be prepared to meet the requirements of Section 926.19, the maintenance agreement shall be prepared to meet the requirements of Section 926.25, and fees shall be those established by the City of Loveland.
