701.04 APPLICATION; REQUIREMENTS; ISSUANCE; DATE AND DURATION.
   (a)   Application. An application for a Certificate of Contractor Registration shall be filed each year with the City Manager or assignee. No certificate shall be issued to any person knowingly making a false statement or misrepresentation in the application.
An application may be filed no sooner than December 1 of the year preceding the period of registration.
   (b)   Requirements. Contractors must provide proof of insurance before they may be licensed. All original applications for licenses, unless otherwise specifically provided, shall be made in writing upon forms to be furnished by the City Manager or assignee and shall contain:
      (1)   The name, address, and business telephone number, and federal tax identification number of the applicant, type of ownership (i.e. sole proprietorship, partnership, or corporation) and the responsible parties for payroll certifications and legal notification.
      (2)   Contractors providing HVAC, sprinklers, plumbing, electrical, or underground fire service line services must also provide a copy of their current license from the state of Ohio.
      (3)   The names of municipalities within the County of Stark or contiguous counties where the applicant has done work.
   (c)   Issuance. Upon receipt of application for the certificate of registration, and compliance with the requirements the City Manager, following verification of the requirements, shall deposit the fee into the appropriate account for the City and issue to the applicant a proper certificate signed by the City Manager.
   (d)   Date and Duration. A license shall not be valid beyond the expiration date therein specified and, unless otherwise provided, shall not extend beyond December 31 for the year issued. Licenses may be renewed annually at the renewal rate before January 1 upon application made to the City Manager or other issuing authority and payment of the prescribed fee. Applications submitted after January 1 will be charged for the full annual license fee. Unless otherwise specified, the full annual license fee will be required of licensees irrespective of the date of issue of such license.
   (e)   Renewal of an annual license may be granted to a licensee in good standing upon the original application, unless otherwise provided. With each original or renewal application, the applicant shall deposit the fee required for the license requested.
      (1)   Liability insurance certificate with coverage limit of not less than one million dollars ($1,000,000) per occurrence for personal bodily injury and property damage.
      (2)   Workers Compensation Insurance Certificate.
