Application for the permit provided for in Section 943.02 shall be in writing, directed and delivered to the City Manager. It shall be dated and signed by the owner or a joint owner of the property to be served, and shall designate the street and number of the building and lot, the owner or owners of the property, the nature and approximate amount of sewage to be accommodated, together with a detailed description or drawing of the location of the premises and the location of the joint where the tap is to be made. The application shall also contain the name of the licensed plumber who is to do the work of tapping.
