A person shall apply to the City Finance Officer for each license required by this subchapter no less than 30 days before the date the tax is due. The application, which shall be submitted on forms provided by the City Finance Officer, shall contain:
(A) The name of the applicant and whether the applicant is an individual, a partnership, a corporation, or some other entity;
(B) The nature of the business;
(C) Where the business is conducted;
(D) An address where notices and statements may be mailed as required by this subchapter;
(E) Whether the business is regulated by a state occupational licensing board subject to G.S. Ch. 93B, and if so, the serial number of the state license the applicant currently holds; and
(F) Any other information the City Finance Officer determines to be necessary to compute the amount of tax due.
