711.04 APPLICATION; FEE.

Application or permit shall include name of applicant who shall be a resident of Lewisburg, Ohio, address, telephone number, nature of merchandise to be sold, date, hours and duration of sale. A fee of one dollar ($1.00) shall be charged for each permit.
