§ 113.061 APPLICATION PROCEDURE.
   (A)   Any person desiring a license to operate a tavern shall file his or her application in writing with the City Clerk-Administrator.
   (B)   Every application shall be made on a form supplied by the city and shall state:
      (1)   The name of the applicant;
      (2)   His or her age;
      (3)   His or her address;
      (4)   Representations as to his or her character with such references as may be required;
      (5)   His or her citizenship, the exact location of the premises upon which the applicant proposes to operate a tavern;
      (6)   Whether the applicant has ever been engaged in any similar business and, if so, the location thereof and the dates when so engaged;
      (7)   Whether the applicant proposes to permit dancing and whether vaudeville entertainment will be permitted. In case the applicant proposes to permit dancing, the applicant shall set forth the amount of floor space which the applicant proposes to maintain for dancing purposes; and
      (8)    Such other information as the Council may require from time to time.
   (C)   It shall be unlawful to make any false statement in an application.
