§ 71.62 PERMIT.
   (A)   Application. All permits shall be issued for a specific Golf Cart in the name(s) of the applicant(s) (Permittee(s)). An application for a permit shall be submitted to the city on a form provided by the city and shall contain the following information:
      (1)   Name(s) and address of applicant(s).
      (2)   Model name, make, year of manufacture, and serial number, if applicable of the Golf Cart.
      (3)   Evidence of Insurance.
      (4)   Other information as the city may require.
   (B)   Conditions.
      (1)   The applicant(s) must be a Resident of Lake St. Croix Beach.
      (2)   The applicant(s) must be at least 18 years of age.
      (3)   The applicant(s) must be a legal Owner of the Golf Cart.
      (4)   The applicant(s) must provide evidence of insurance complying with the provisions of Minnesota Statutes concerning insurance coverage for the Golf Cart. Proof of insurance shall be maintained on, or within, the Golf Cart at all times that it is Operated on city Streets.
      (5)   The applicant(s) for a Golf Cart permit and all Operators of the Golf Cart on city roads must have a valid driver's license.
      (6)   The applicant(s) has not had their driver's license revoked as the result of criminal proceedings.
   (C)   Permit fees and term.
      (1)   The fee for such permit shall be in accordance with the current city fee schedule, as amended.
      (2)   Permits will be granted for a one-year period. Permits may be purchased at any time during the year but will be valid only for one calendar year, including the year of issuance and will expire on December 31 of the year of issuance.
      (3)   The permit may be suspended or revoked by the City Clerk-Administrator or Law Enforcement upon finding that a permit holder has violated the conditions of this ordinance or state statute, has abused the privilege of being a permit holder, or upon finding that the Permittee cannot safely Operate the Golf Cart on the designated Roadways. There will be no refund of the permit fee if suspended or revoked. If a permit is revoked by the city the Permittee(s) may reapply for a new permit twelve (12) months after the date of revocation.
      (4)   Upon receipt of application and fee, Permittee(s) will be issued a numbered sticker to affix to the left side rear fender or similar component of the Golf Cart.
