Upon receipt of the documentation required under § 37.06, the Office of the City Controller is authorized to rely upon the accuracy of the information contained in such documentation and act upon the basis of the same. Should the member change his or her military Power of Attorney or should the member choose to change his or her designated recipient, it shall be the responsibility of the member to advise the Office of the Controller of the change in writing.
