249.25 CITY’S CONTRIBUTION TO HOSPITALIZATION; LIFE INSURANCE; ALLOWANCES.
   (a)   Hospitalization. Only full-time employees in active pay status or part-time employees that work between twenty-five and thirty-five hours per calendar week in active pay status may be eligible to participate in the employer’s health insurance program. The City agrees to contribute eighty-five percent of the monthly premium rate for family and/or single coverage. For part-time employees working between twenty-five and thirty-five hours per week, the City shall not contribute to such premium rate and such part-time employee shall be responsible for paying 100 percent of the monthly premium rate for family and/or single coverage if such part-time employee elects to participate in such plan, unless such City contribution is required by State or Federal law, in which case such contributions shall be made to the City only to the extent required by such laws. As used herein, the terms “full-time employee” and “part-time employee” are defined by their individual job descriptions or Section 249.02 , 249.03 , and 249.13 .
   (b)   Life Insurance. The City shall provide a fifty thousand ($50,000) group term life insurance policy covering all full-time employees (as defined by their individual job descriptions or the City’s Codified Ordinance Section 249.13 ).
   (c)   Allowances.
      (1)   The following rates of reimbursement are established for City officials or employees while on City business:

      (2)   Reimbursement for the use of any employee of the City of his or her personal vehicle for City business shall be on a mileage-allowance basis only at such rate as is established in this subsection. Each such employee shall keep an accurate daily record of such mileage, for which reimbursement may periodically be made.
