Sec. 8-49.  Telecommunication And Fiber Optic Installations:
   (a)   Permit Required: It shall be unlawful to construct or erect any portion of a telecommunication or fiber optic system prior to obtaining a permit from the village. An application shall be filed with the office of the building commissioner upon such forms as he shall deem necessary. The application shall be accompanied by a complete set of plans showing the location of all work to be performed, a copy of the contract reflecting the total cost of the project and the linear feet covered; and the certification of a licensed electrical contractor that the plans and specifications comply with the applicable village electrical ordinances.
   (b)   Contractors: Any and all persons constructing or erecting a telecommunication or fiber optic  system shall obtain a license as a general contractor under section 8-45 of this code.
   (c)   Occupation Of Streets: No street shall be blocked or occupied by any person constructing or erecting any portion of a telecommunication or fiber optic system without obtaining a street occupation permit. For the purposes of telecommunication or fiber optics, the locations to be blocked or occupied shall be given to the building commissioner and the public works director at least two (2) business days prior to their blockage or occupation. Adequate warning devices, including barricades, shall be supplied to direct traffic to other areas by the contractor.
   (d)   Permit Fee: The permit fee for the construction and erection of a telecommunication or fiber optic system shall be one percent (1%) of the total cost of the project attributable to the village of Justice.
   (e)   Restoration: All disturbed areas within the village due to a telecommunications or fiber optic installation shall be restored back to its original condition within ninety (90) days of the completion of the project. Failure to restore any disturbed area shall be subject to a fine not to exceed seven hundred fifty dollars ($750.00) for any one offense. A separate and distinct offense shall be held to have been committed each day any person or entity continues to violate any of the provisions hereof.
