(a) At the time a taxpayer secures a building permit for the construction of the improvement for which an exemption is requested, the taxpayer shall apply to the Director of Finance for an exemption. A request for an exemption shall be in writing and certified on forms prescribed by the Director of Finance setting forth the following information:
(1) The date the building permit was issued for such improvement;
(2) The type of improvement;
(3) A summary of the plan of the improvement;
(4) The cost of the improvement; and
(5) Such additional information as the Director of Finance may require.
