The license application referred to in the preceding section shall contain the following information:
(a) Satisfactory evidence that the towing contractor owns or controls vehicles and equipment capable of towing and handling automobiles, light trucks, all-terrain vehicles and two-wheeled vehicles in such manner as to avoid damage to such vehicles.
(b) Satisfactory evidence that the towing contractor has such special license plates as are necessary under the provisions of the Ohio Revised Code to tow unlicensed motor vehicles.

(c) Satisfactory evidence that the towing contractor has obtained liability insurance consisting of the amount as determined by the Village Council for combined single limit liability coverage and for the sum as determined by the Village Council for on-hook towing coverage, naming the Village of Johnstown, Licking County, Ohio, as a co-insured. The application must contain a certificate of insurance showing that such coverage is in effect and that such coverage will remain in effect for the remainder of the license period.

(d) Satisfactory evidence that the towing contractor has an established fee schedule which will remain constant for the term of the license, which fees include towing fees, daily storage fees and any other fees or charges.
(e) Satisfactory evidence that the towing contractor will guarantee a response time of one hour from the time that the towing contractor is contacted until the towing contractor arrives to tow the motor vehicle.
(f) Satisfactory evidence that the towing contractor has a secured storage area in which to store all motor vehicles placed into storage.
(g) Satisfactory evidence that the towing contractor has a valid Ohio operator's license.
(h) Such other information as the Manager may require.
