(a) Application for a license for a mechanical amusement game or mechanical musical device or devices shall be made to the Manager on forms provided by him and shall set forth the name, residence and business address of the exhibitor of the device and the place where the device is to be exhibited.
(b) The Manager shall cause an investigation to be made of the applicant for a license, including the applicant's moral character, and of the premises wherein the games and devices are to be operated. The Manager shall have authority to approve or reject any application for a license.
