5-2-6: LICENSE, PLAN REVIEW AND CHANGE OF OWNERSHIP FEES:
   A.   License Fees Required: A license fee in the amount set by the Jo Daviess County Board of Health and approved by resolution of the Jo Daviess County Board must be submitted along with the completed application form and accompanying paperwork.
   B.   Proration Of Fees: When application is made and license is issued during the first six (6) months of the fiscal year, the license fee shall not be prorated. When application is made and a license is issued during the last six (6) months of the fiscal year, the license fee shall be one-half (1/2) of the fee. The benefit granted by this applies to the initial annual category I, category II, or category III license only. This benefit does not include applications for seasonal or temporary licenses.
   C.   Plan Review: When a plan review is deemed necessary, a plan review fee, as set by the Jo Daviess County Board of Health and approved by resolution of the Jo Daviess County Board, is required to be submitted along with plans. This fee must be paid in full before the preoperational inspection will be performed.
   D.   Change Of Ownership: If a new license is required because of a change of ownership, and no changes are being made to the menu, structure, or equipment of the food establishment, a change of ownership fee is required to be submitted, in addition to the applicable license fee, with a completed application form provided by the regulatory authority before a pre-operational inspection will be performed and new license issued. No plan review fee is required when a change of ownership fee is submitted. (Ord. 2018-10, 10-9-2018, eff. 12-1-2018)
