A. Any person desiring to operate a food establishment or renew an expired license shall make written application for a license on forms provided by the regulatory authority. Such application shall include the name and address of each applicant, the location and type of the proposed food establishment, and the signature of each applicant. A completed food establishment plan review packet must be filled out for those food establishments that the regulatory authority deems necessary. Any mobile food establishment application must include proof of access to a commissary and a list of all known events and locations at which the operator will be preparing and serving food.
B. Every food establishment shall be assigned a risk classification based on an assessment of the relative potential risks of creating a foodborne illness. The classification will result in the establishment being placed into a license category as it relates to food safety. Criteria in the State of Illinois Food Service Sanitation Code 77 Ill. Adm. Code 750 shall be used to determine the risk classification. The risk classification will be verified annually. (Ord. 2018-10, 10-9-2018, eff. 12-1-2018)
