Every owner or operator required to secure a motor vehicle license as provided in this chapter shall file an application with the village administrator, upon a form provided by the village administrator, setting forth the name and address of the applicant, the Illinois vehicle registration number, the vehicle identification number, a description of the motor vehicle and such other information relative thereto required by the chief of police.
