§ 113.63 CERTIFICATION IDENTIFICATION CARD.
   (A)   A person may be issued a food service manager's certification card if he meets the requirements set forth in § 113.62. The card shall include:
      (1)   The applicant's full name and post office address.
      (2)   Identifying features such as height, weight, and age.
      (3)   The signature of the person certified.
      (4)   The signature of the Director.
      (5)   The date issued.
      (6)   The expiration date.
      (7)   A photograph of the holder.
   (B)   A person possessing a food service manager's certification card shall have said card on his person at all times while on duty.
   (C)   A person possessing a food service manager's certification card shall present said card for examination when requested to do so by the Health Officer or his duly authorized representative.
   (D)   Valid food service manager's certification cards remain the property of the Bell County Health Department and may be required to be returned to the Health Officer in case of revocation.
