§ 101.056 APPLICATION FOR GRADE.
   (A)   It shall be unlawful for any person to construct, reconstruct or make a general repair of any sidewalk or make any excavation in any street or sidewalk for the purpose of constructing, reconstructing or making a general repair of any sidewalk without having first applied to the City Engineer in writing for the grade of the sidewalk.
   (B)   It shall be the duty of the City Engineer to furnish the applicant with a blank application for the grade, wherein shall be specified the date of the application, the location of the premises on which the sidewalk abuts by number and street or lot and addition, the kind of sidewalk proposed to be constructed, reconstructed or repaired, and the date when the applicant proposes to begin the work.
   (C)   The application shall be signed by the property owner, and the person or contractor doing the work.
