§ 93.082 SPECIAL INFORMATION TO BE FURNISHED BY PERSONS DEALING WITH HAZARDOUS SUBSTANCES.
   (A)   Any person that uses, stores, handles, or disposes of substances defined as dangerous, hazardous, or toxic by § 93.080, on property zoned I-1 Light Industrial and/or I-2 Manufacturing under Chapter 159, shall be required to furnish the following at his or her location within the city:
      (1)   For all facilities which are not staffed 24 hours a day, a weather-proof lock box of sufficient size to hold a binder or binders that shall contain all the Material Safety Data Sheets or other similar substance information sheets for each dangerous, hazardous or toxic substance used, stored, handled or disposed of; and
      (2) A facility plot plan, or other plans and documents, made to the scale suitable for the purposes indicated below. The weather-proof lock box shall be bright yellow in color on all sides, including the top and bottom, and meet ANSI (American National Standards Institute) Specification 253.l. For all facilities which are continuously staffed, the information shall be available at a specified staffed entrance. The Fire Chief or his or her designee shall have the discretion to modify any parts of this section.
   (B)   Information on the plot plan, or other plans and documents shall include:
      (1)   The location and identification of each structure;
      (2)   The location and size of all fire hydrants located on the premises, and within 500 feet of the boundary line of the premises; and
      (3)   The location and quantity of all major firefighting, spill response or safety-related equipment such as but not limited to: water sprinkler systems; foam suppressants (by type and quantity); dry powder suppressants (by weight or size); protective clothing (specify fire/acid suits or other special suits); self-contained breathing apparatus (list amount of breathing time in minutes); resuscitators; natural gas shutoff valves; electric shutoff points; municipal water shutoff points; drains to the Hammond Sanitary District; inplant telephones; other telephones; and other items the Fire Chief or designee may require.
   (C)   The lock box shall be located within 100 feet of the entrance to the plant or facility, at a height of between 36 inches to 48 inches above the ground. The placement of the lock box in the plant or facility shall be approved by the Chief of the Fire Department or his or her designee. Four keys to the lock box shall be provided to the Chief of the Fire Department for use solely by the Fire Department in the event of a spill, leak, discharge or release.
   (D)   An Emergency Action Plan (EAP) that shall outline the responsible persons and their actions in the event of a fire, spill, leak, discharge or release of a dangerous, hazardous or toxic substance regulated by this section shall be provided.
