(A) Any person eligible by virtue of a physical handicap for special privileges under this subchapter shall present to the Street Department a written application, on a form furnished by the Street Department, for an address-specific permit sticker, entitling that person to the exclusive use, for a period of 2 calendar years from date of issue, of 1 parking space adjacent to the applicant’s place of residence. This application shall include a current certificate from a physician verifying the applicant is qualified for a handicapped parking space. If upon the expiration date of the permit the handicap still exists, the permit may be renewed. If the handicap is no longer in existence, the permit holder shall remove the sticker from the vehicle and return the sticker to the Street Department so that any and all signs or curb markings may be removed.
(B) While the permit is valid and the permit holder changes vehicles, it shall be the responsibility of the permit holder to remove the permit sticker from the original vehicle and return the sticker to the Street Department for issuance of a new, replacement sticker.
(C) The Street Department is authorized to revoke any permit it has issued or to remove any parking sign from any residence that becomes vacant or if the qualifying person no longer resides at that address.
