335.15 ACCIDENTS REQUIRING POLICE REPORTS.
(a) The driver of a vehicle involved in an accident as defined in Ohio R.C. 4509.01 shall, within fifteen days after such accident, forward a written report of such accident to the Police Division of this City.
(b) The Police Division may require any driver of a vehicle involved in an accident of which a report must be made as provided in this section, to file supplemental reports whenever the original report is insufficient in the opinion of the Division with reference to the time, date, place, nature of the accident, weather conditions, road conditions, name and address of the persons, and ownership of the vehicles.
(c) Whenever the driver of a vehicle is physically incapable of making a required accident report and there was another occupant in the vehicle at the time of the accident capable of making a report, such occupant shall make or cause to be made the report.
(d) Whoever violates any provision of this section is guilty of a minor misdemeanor on a first offense; on a second offense within one year after the first offense, the person is guilty of a misdemeanor of the fourth degree; on each subsequent offense within one year after the first offense, the person is guilty of a misdemeanor of the third degree.
