172.04 APPLICATION.
   (a)   Every entity seeking a grant shall submit a Grant Application Form to the Director of Finance on a form provided for that purpose, which shall include, at a minimum, the following information:
      (1)   The entity’s name, address, telephone number and contact person;
      (2)   A brief description of the entity’s history, purpose, accomplishments and goals;
      (3)   A brief description of one program to be funded including, but not limited to, the number of people served, the geographical area and how success will be measured;
      (4)   An itemized budget;
      (5)   Proof of IRS certifying 501(c)(3) status; and
      (6)   A copy of financial statements or statement of income and expenditures and assets and liabilities, if available.
   (b)   Grant applications shall be required to be submitted prior to the date designated by the Finance Director each year. Such date shall be designated annually by the Finance Director and shall not be before July 1st nor after November 15th.
   (c)   Grant applications received before the designated date shall be considered for the following budget year. Grant applications received after the designated date shall not be considered.
   (d)   Requests shall not be considered from organizations that discriminate on the basis of race, creed, color, age, gender, religion, national origin, or veteran status.
   (e)   The City shall not accept proposals from individuals, or from fraternal, political, lobbying or religious organizations.
   (f)   The Director of Finance may, from time to time, modify any of the particulars of the application, upon approval of Council.
