911.09 STATEMENT AND MAP.
   It shall be the duty of every person owning, claiming or using as his own any pole or post located in any street or thoroughfare, or in any park or other public grounds of the City, within three months after the passage of this section, to furnish to the Director of Public Utilities a list or statement of all poles or posts erected, set or maintained, or used as his own by him, stating or showing the location, height and purpose of each, and the nature and number of wires, cables or other fixtures or appurtenances supported by each. Such list or statement shall be accompanied by a map of the City or of so much thereof as may be affected, on which the location of each pole or post shall be exactly located to the satisfaction of the Director of Public Utilities, and to which can be added the location of poles thereafter erected or acquired. It shall likewise be the duty of such person, whenever any change is made in the number of wires, cables, fixtures or appurtenances carried by any pole or post, to notify the Director of Public Utilities of the changes so made.
